Once we receive the reservation form we will give you your ticket(s) to the banquet.
If you participated in any sport at any level this school year you are invited to the banquet.
The cost of the banquet is $15 for student athletes and $20 for all other guests.
We ask that you please return these reservation forms with payment to the receptionists desk no later than Friday, April 29, 2016.
